The Sole Genie
is required to hold certain information relevant to your treatment including:
- your name
- contact details including telephone number and email
- personal information such as date of birth and occupation
- assessment results
- medical history
- medication details
Each treatment is also recorded and any verbal advice that's given in person or over the telephone is also held on record.
Contact details are needed to re-arrange, cancel your appointment or to make a medical follow up call (for example check a dressing).
Medical history and assessment results are needed for your benefit, as its very important to carry out a continued and professional treatment.
This data
is kept on your patient treatment card which is securely locked away.
Personal details
will not be processed or accessed by any other persons other than Helen Millins at The Sole Genie.
Details will not be passed on to any third parties.
In the event of a medical emergency, a GP can be contacted and any emergency service may access clinical records in such an event.
Details will be retained for a period of up to 7 years after the last appointment date.
